Assisted Living

When selecting an assisted living community for your loved one, it’s important to consider how their needs and care will be evaluated.  At Windemere Park of Oakland we offer assisted living programs that encourage Residents to live independently. We are committed to providing your loved one with a level of care that is customized to their specific needs.

Assisted Living Care That Adapts to Changing Needs

A professional medical and care assessment is made prior to admission, upon move in, and periodically throughout their stay at Windemere. Changes in a Resident’s needs, behaviors and physical condition are continually observed and documented. Windemere Park of Oakland assures the appropriate level of care is available to every Resident, adhering to the following levels of care definitions:

Levels of Care

– State Licensed Facility –

Level I

Independent Plus:

Residents are fully functionally independent or are mentally alert and require minimal assistance with mobility, dressing, grooming, and bathing. We offer nutritious meals (including special diets), assistance with housekeeping, laundry, as well as stimulation and the security of not living alone. Medications, if any, are administered by trained Windemere Park staff. Significant changes in a Resident’s condition are documented by the Windemere Park staff.

Level II

Assisted Care:

Residents’ needs are the same as Level I, yet require additional staff assistance. Medications may require special monitoring, i.e., blood work, frequent changes in physician orders for unstable medical conditions. Routine weekly or semi-weekly assistance is required with one or more life enrichment of daily living such as bathing, grooming, toileting, ambulation, transfers, and dining.

Level III

Supportive Care:

Residents need 24-hour staff availability and may require involvement of other professionals to accommodate their needs.  Residents at this level require significant daily assistance with one or more of the following:

  • Dressing, grooming and/or bathing
  • Toileting/incontinence management
  • Transferring or mobility
  • Behavior management
  • Cueing to meals and/or assistance with dining

Level IV

Extensive Care:

Significant on-going symptoms requiring continuous observations and frequent adjustments to care plan:

  • Requirement of supervision/cueing to meals
  • Requirement of planned intermittent toileting intervention
  • Catheterization or colostomy care
  • Resistant to routine bathing / dressing
  • One-person physical assistance for transferring and mobility, or assistance if one does not request help and has fallen more than twice in the last 30 days
  • Wound care supervision and treatment
  • Frequent monitoring and/or lab work for effectiveness or side effects required by medication regimen.
  • Provision of special building security measures and/or increased monitoring and redirection

Assisted Living Services:

  • Three Chef-prepared nutritious meals with snacks
